Transdural herniation of the cervical spinal cord as a complication of a broken fracture-fixation wire.

چکیده
Herniation of the spinal cord through a dural rent is a rare occurrence. Although sporadic cases have been documented, the diagnosis has never been made preoperatively [1 -3J . We report a case of a transdural cord herniation at the C1 C2 level. The dural tear was a result of penetration by a broken fracture-fi xation wire and its subsequent complications. Transdural spinal cord herniation was demonstrated preoperatively by CT metrizamide myelography and MR imaging. Although conventional myelography did not demonstrate the hernia directly, it did reveal characteristic myelographic findings. To our knowledge, the complication of a broken fracturefi xation wire penetrating the dura has not been previously reported. Tears in the spinal dura require surgical closure to prevent nerve root entrapment, meningitis, or cord herniation .

Intrapelvic Protrusion of a Broken Guide Wire Fragment during Fixation of a Femoral Neck Fracture
During fixation of a femoral neck fracture in a 23-year-old male patient with cannulated screws, a broken guide wire fragment inadvertently advanced through the hip joint and protruded into the pelvis. A second surgical approach was needed to remove the broken fragment from the pelvis. Awareness of such a potentially devastating complication will make surgeons more cautious during implementa...
متن کاملIntrapelvic Protrusion of a Broken Guide Wire Fragment during Fixation of a Femoral Neck Fracture
Breakage of DHS guide wire during surgery and its migration into the pelvis through the hip joint is a rare complication and its removal can be very challenging for the surgeon. We share our experience of a similar case wherein we used an ‘iliofemoral’ approach to successfully remove the broken transfixing guidewire from the hip joint. Although iliofemoral approach is similar to the lateral win...
متن کاملLate migration of threaded wire (schanz screw) from right distal clavicle to the cervical spine.
We report a 49-year-old man who had undergone osteosynthesis to treat right distal clavicular fracture with a threaded wire (Schanz screw). The wire could not be removed due to its firm fixation within the bone. Eight years later, migration of the broken wire to the right 7th cervical vertebra punctured the lamina, with no spinal cord injury noted. The threaded wire was extracted from the C7 la...
متن کاملThe Cervical Sequestrated Disc Fragment Presenting as Mass Lesion: Case Report and Review of the Literature
Background & Importance: Disc herniation is defined as the prolapsed of material disc into the spinal canal. If the disc fragment is free in spinal canal, it is called sequestration disc fragment. Occasionally, alteration in sequestration disc fragment makes the diagnosis of the disc herniation, challenging. Case Presentation: We reviewed a rare case of the cervical sequestrated disc...
متن کاملTransdural spinal cord herniation: imaging and clinical spectra.
PURPOSE Transdural herniation of the spinal cord is a rarely reported clinical entity, and many of the existing reports were published before the advent of MR imaging. We describe five current cases and compare them with findings in 25 cases reported in the literature to delineate the clinical and imaging spectra of transdural spinal cord herniation. METHODS MR imaging, CT myelography, and co...
